Hydromorphone (하이드로몰폰)

  • 적응증: 중등도~중증 통증

  • 분포: BBB 통과, 전신
  • 대사/배설: 간 대사, 신장 배설
  • 부작용: 호흡 억제, 변비, 오심
  • 용량: 2~4mg/4~6시간마다
  • 간호 포인트: 호흡수 확인 필수, 중독 가능성 주의

 

HYDROmorphone (Dilaudid): Allergy, cold, cough remedies(antitussives), opioid

Class Opioid analgesic 진통제 / antitussive 기침을 억제하는 ; opioid agonist
Indications - Moderate to severe pain
- Antitussive (in lower doses)
Therapeutic Effects - Relief of moderate to severe pain
- Suppression of cough
Absorption Well absorbed via PO, rectal, subcutaneous (Subcut), and intramuscular (IM) routes
Distribution Widely distributed; crosses placenta and enters breast milk
Protein Binding ~20%
Metabolism & Excretion Metabolized primarily by the liver; excreted via urine
Adverse Drug Effects (ADE) CNS: Confusion, sedation
CV: Hypotension
GI: Constipation
Dose - PO ≥50 kg: 4–8 mg every 3–4 hrs as needed
- PO <50 kg: 0.05 mg/kg every 3–4 hrs
- IV/IM/Subcut ≥50 kg: 0.06 mg/kg q3–4h
- <50 kg: 0.015 mg/kg q3–4h
Considerations Monitor for respiratory depression, sedation, and constipation; assess pain before and after administration; use with caution in opioid-naive patients
