Quetiapine (Seroquel)

• 적응증: 조현병, 양극성 장애, 우울증 보조요법
• 작용 기전: 도파민 및 세로토닌 수용체 길항
• 흡수와 대사: 장에서 흡수, 간 CYP3A4 대사, 신장 배설
• 부작용: 졸음, 체중 증가, 저혈압, QT 연장
• 투여량: 초기 50mg, 증량 후 300–800mg/일
• 간호 고려사항: 활력징후 모니터링 (혈압, 맥박), ECG 모니터링, 졸림으로 인한 낙상 주의

• 부작용: 졸림, 체중 증가, 기립성 저혈압
• 간호 포인트: 자살 경향 모니터링, 기립성 저혈압 예방 위해 천천히 일어나기

QUEtiapine (SEROquel) [kwuh tai ah peen] 

Antipsychotics, mood stabilizers 

● Indications: schizophrenia, depressive episodes w/ bipolar disorder, acute manic episodes associated w/ bipolar disorder, maintenance treatment of bipolar 1 disorder, adjunctive treatment of depression 

● Therapeutic effects: decreased manifestations of psychoses, depression, or acute mania 

● Absorption: well ac PO 

● Distribution: wide 

● Metabolism&Excretion: by liver, <1% unchanged in urine 

● ADE: CNS; neuroleptic malignant syndrome, seizures Derm; drug reaction w/ eosinophilia and systemic symptoms, stevens-johnson syndrome GI; gi obstruction, pancreatitis Hemat; agranulocytosis, decreased Hb

● Dose: PO; 25 mg BID on day1, up by 25-50 mg 2-3/day on day2 and 3, up to 300-400 mg/day in 2-3 divided by day 4 

Quetiapine (Seroquel)

• Indications: Schizophrenia, bipolar disorder, major depressive disorder (adjunctive therapy).
• Therapeutic Effects: Antagonist at multiple neurotransmitter receptors, including serotonin and dopamine.
• Absorption: Well absorbed orally; peak plasma concentration in 1.5 hours.
• Distribution: Extensive distribution; 83% plasma protein binding.
• Metabolism & Excretion: Hepatic metabolism via CYP3A4; excreted in urine and feces.
• Adverse Effects: Sedation, weight gain, orthostatic hypotension, dry mouth.
• Dose: Initial: 50 mg/day; titrated to 300–400 mg/day in divided doses.
• Considerations: Monitor for metabolic syndrome; assess for extrapyramidal symptoms.MedscapeMedscape
