Fong’s Disease (Pachyonychia Congenita) – English

Definition:
A rare, inherited genetic disorder that affects keratin production, leading to thickened nails, skin changes, and painful calluses.

Cause:

• Autosomal dominant inheritance
• Mutations in keratin genes (KRT6A, KRT6B, KRT16, KRT17)

Clinical Manifestations:

• Thickened toenails & fingernails (onychogryphosis)
• Painful plantar keratoderma (thick skin on soles)
• Blistering on feet
• Follicular hyperkeratosis
• Oral leukokeratosis (white patches)

Complications:

• Severe walking pain
• Secondary infections
• Mobility limitation

Treatment (No cure, supportive only):
Non-pharmacological

• Proper foot care
• Nail filing/trimming
• Avoid friction, pressure
• Use cushioned footwear

Medications

• Keratolytics: urea, salicylic acid
• Oral retinoids (severe cases)
• Pain management

Nursing Considerations

• Monitor for infection
• Promote mobility comfort
• Support coping (chronic painful condition)

정의:
케라틴(keratin) 유전자 이상으로 인해
손발톱 비후, 발바닥 각질증, 물집 등이 생기는 드문 유전 질환.

원인:

• 상염색체 우성 유전
• KRT6A, KRT6B, KRT16, KRT17 유전자 변이

임상 증상:

• 손발톱 비후
• 발바닥 각질 증가 및 통증
• 발 물집
• 모낭 각화
• 구강 백색판(하얀 패치)

합병증:

• 보행 시 심한 통증
• 2차 감염
• 이동 제한

치료 (완치 없음, 증상 관리):
비약물적

• 발 관리, 발톱 손질
• 압력·마찰 최소화
• 쿠션 신발 착용

약물적

• 요소연고, 살리실산 등 각질용해제
• 경구 레티노이드 (심한 경우)
• 통증 조절

간호 중재

• 감염 징후 확인
• 통증 및 이동성 관리
• 평생 관리법 교육
• 심리적 지지 제공
