Anthrax Infection (탄저균 감염)
1. Definition (정의)
English:
Anthrax is a serious bacterial infection caused by Bacillus anthracis, which forms spores that survive in the environment for years. It commonly affects animals but can infect humans through skin contact, inhalation, or ingestion.
2. Types of Anthrax (탄저의 종류)
1) Cutaneous Anthrax (피부 탄저) — Most common (90%)
- Enters through a skin cut
- Painless black eschar (charcoal-like scab)
2) Inhalation Anthrax (흡입 탄저) — Most deadly
- From inhaling spores
- Severe respiratory failure, shock
3) Gastrointestinal Anthrax (위장관 탄저)
- From eating contaminated meat
- Severe abdominal pain, bloody diarrhea
4) Injectional Anthrax (주사 관련 탄저)
- Seen in IV drug users
- Deep tissue infection
3. Symptoms (증상)
Cutaneous (피부)
- Itchy bump → blister → black necrotic eschar
- Swelling
- Low-grade fever
Inhalation (흡입)
Early:
- Fever, cough, malaise
- Chest discomfort
Late (rapid deterioration): - Severe dyspnea
- Mediastinal widening
- Shock, sepsis
Gastrointestinal (위장관)
- Nausea, vomiting
- Severe abdominal pain
- Bloody diarrhea
- Fever
4. Transmission (전파 경로)
- Contact with infected animals or animal products
- Inhaling spores
- Eating contaminated meat
- NOT spread person-to-person (usually)
Korean:
- 감염된 동물/가죽 제품 접촉
- 포자 흡입
- 오염된 고기 섭취
- 사람 간 전파는 거의 없음
5. Diagnosis (진단)
- Culture (blood, lesions)
- PCR
- Chest X-ray/CT (inhalation)
- Serology
6. Treatment (치료)
Antibiotics (항생제)
First-line agents:
- Ciprofloxacin (1st choice)
- Doxycycline
- Penicillin (if susceptible)
Severe cases:
- Combination therapy (e.g., Cipro + Clindamycin + Rifampin)
Antitoxin (항독소)
Used in inhalation anthrax or systemic disease:
- Raxibacumab
- Anthrax immune globulin (AIG)
7. Supportive Care (지지적 치료)
- IV fluids
- Oxygen therapy
- Vasopressors if shock
- Mechanical ventilation if respiratory failure
8. Nursing Considerations (간호 중재)
English:
- Standard precautions (anthrax is not usually contagious)
- Monitor airway/breathing (especially inhalation anthrax)
- Assess skin lesions (cutaneus)
- Maintain strict adherence to long antibiotic therapy (60 days)
- Report to public health authority (mandatory)
- Monitor for sepsis, shock
9. Prevention (예방)
- Animal vaccination
- Avoid contact with potentially infected animal hides
- Anthrax vaccine for high-risk occupations (military, lab workers)
