Abdominal Cholecystectomy (Open Cholecystectomy)

 

1. Definition (정의)

English:

An abdominal cholecystectomy, also called an open cholecystectomy, is a surgical removal of the gallbladder through an incision in the abdomen. It is typically performed when laparoscopic surgery is not safe or feasible.

Korean:

**복부 담낭절제술(개복 담낭절제술)**은 복부를 절개하여 담낭을 제거하는 수술입니다. 복강경 수술이 어렵거나 위험할 때 시행됩니다.

2. Indications (적응증)

English:

• Acute cholecystitis (급성 담낭염)
• Gallstones (담석증)
• Gallbladder perforation
• Severe infection / sepsis
• Pancreatitis due to gallstones
• Anatomical issue preventing laparoscopic surgery

• 급성 담낭염
• 담석증
• 담낭 천공
• 중증 감염/패혈증
• 담석으로 인한 췌장염

3. Procedure (수술 과정)

English:

• A 10–20 cm incision is made in the right upper abdomen.
• Gallbladder is identified and removed.
• Cystic duct and cystic artery are clipped and cut.
• Drain may be placed.
• Incision closed with sutures/staples.

• 우상복부에 10–20cm 절개
• 담낭 확인 후 제거
• 담관과 담낭 동맥을 결찰 후 절단
• 필요시 배액관 삽입
• 봉합 혹은 스테이플러로 마무리

4. Post-Op Concerns (수술 후 주의사항)

• Pain management
• Watch for infection at incision site
• Monitor drainage (if JP drain used)
• Encourage early ambulation
• Deep breathing & incentive spirometer
• NPO → clear liquids → regular diet as tolerated

• Keep incision clean and dry
• Report fever, redness, or yellow drainage
• Eat small, low-fat meals at first
• Gradually resume normal diet

• 발열, 발적, 황색 분비물 즉시 보고
• 초기에는 저지방 작은 식사
• 점차 일반식으로 회복
