Indications / 적응증

• English:
  • Relief of acute bronchospasm in asthma, COPD, bronchitis
  • Prevention of exercise-induced bronchospasm
• Korean:
  • 천식, COPD, 기관지염의 급성 기관지 경련 완화

  • English: Paradoxical bronchospasm, severe hypertension, arrhythmias

Dosage / 용량

• Inhalation (Adults): 2 puffs every 4–6 hrs as needed
• Korean: 흡입 (성인): 필요 시 4–6시간마다 2회 흡입

 

 

 

 

 

 

Nursing Considerations / 간호 시 고려사항

• English:
  • Monitor respiratory rate, lung sounds, oxygen saturation
  • Assess for paradoxical bronchospasm
  • Use bronchodilator before corticosteroids when both are prescribed
• Korean:
  • 호흡수, 폐음, 산소 포화도 모니터링
  • 역설적 기관지 경련 여부 확인
  • 기관지 확장제는 스테로이드제 흡입 전 먼저 사용

 

 

 

 

 

 

Patient Teaching / 환자 교육

  • Use inhaler correctly (demonstrate technique)
  • Do not exceed prescribed dose
  • Rinse mouth after inhalation to reduce throat irritation
  • Report chest pain, irregular heartbeat, worsening wheezing
• Korean:
  • 흡입기 사용법을 정확히 익히세요
  • 처방된 용량을 초과하지 마세요
  • 흡입 후 입을 헹궈 인후 자극을 줄이세요
  • 흉통, 불규칙한 심장 박동, 천식 악화를 보고하세요

Bronchodilators / adrenergics

● Indications:  treatment or prevention of bronchospasm in asthma or chronic obstructive pulmonary disease (COPD), inhalation : Prevention of exercise-induced bronchospasm 운동 유발성 기관지 경련 예방

● Therapeutic effects: Bronchodilation 기관지 확장제제

● Absorption: Well absorbed after PO, rapidly undergoes extensive metabolism.

● Distribution:  Small amounts appear in breast milk

● Protein binding: 

● Metabolism&Excretion: by the liver and other tissues

● Onset/Peak/Duration: PO: Onset 15-30mins, Peak 2–3 hr, Duration 4-6 hr or more, injaln: Onset 5-15 min, Peak 60-90mins, Duration 3–6 hrs

● ADE: CV: chest pain, palpitations, angina, arrhythmias, hypertension. Endo: hyperglycemia. F and E: hypokalemia. GI: nausea, vomiting. Neuro: nervousness, restlessness, tremor, headache, hyperactivity (children), insomnia. Resp: PARADOXICAL BRONCHOSPASM (excessive use of inhalers)

● Dose: Adult: PO: 2– 4 mg 3– 4 times daily (not to exceed 32 mg/day) Inhaln:NIH Guidelines for acute asthma exacerbation via nebulization or IPPB—2.5– 5 mg every 20 min for 3 doses then 2.5– 10 mg every 1– 4 hr as needed; Continuous nebulization—10– 15 mg/hr

● contraindication: Monitor lung sounds, respiratory status, oxygen saturation before/after administration,Monitor pulmonary function tests before initiating therapy and periodically during therapy, Observe paradoxical bronchospasm (wheezing). If condition occurs, withhold medication and notify health care professional immediately

● nursing consideration: Hypersensitivity to adrenergic amines
